system.
You will receive a confirmation via email*. please print
the email and forward to the department.

printed copies of the approved thesis (identical to
the pdf version)

additional requirement for
PhD: all PhD students are
required to complete and submit to the department

Electronic Thesis Agreement Form
(p.4-5) (instructions)
for publication of their PhD theses in the ProQuest
database.
